UPDATE ON NECK REJUVENATION

Rejuvenation of the neck is one of the most common reasons I see patients in consultation in my office. Although the surgical facelift and neck lift remain the gold standard for neck rejuvenation, many patients cannot find the time out of their busy schedules to commit to a surgical procedure. For other patients, the results they desire do not require a surgical procedure and may be achievable with minimally invasive and non-invasive techniques.

Evaluation of the neck area and determination of the best procedure for that patient requires looking at the amount of fat in the subcutaneous space as well as the degree of laxity and wrinkling in the skin. For those patients who do not elect or do not require surgery, individualization of the proper clinical plan requires considering multiple devices and procedures. I am happy to announce that we can now offer the PrecisionTx procedure for neck rejuvenation. Our full inventory of neck rejuvenation devices now includes the SmartLipo laser (PrecisionTx), the ThermiTight radiofrequency, the Intensif RF (radiofrequency), ablative and non-ablative lasers, and the non-invasive RF devices (EndyMed, Pelleve, and ThermiSmooth).

The SmartLipo (PrecisionTx) and ThermiTIght procedures both require inserting a small fiber through a needle stick and heating the subcutaneous fat and the undersurface of the skin. Both devices use temperature monitoring in order to operate in a safe zone but still achieve the desired results. Afterwards, the liquified fat will be removed with a small 2.4 mm cannula. These procedures are all done under a local anesthesia. Although the patient will see improvement almost immediately, the final result might not be seen for 3-6 months.

The Intensif radiofrequency devices uses a combination of microneedles and energy in a series of treatments to achieve tightening of the neck. This procedure requires a topical anesthetic such as EMLA cream. Our office was one of the 3 sites where the studies were performed for the published indication for neck tightening using the Intensif.

The non-invasive as well as maintenance procedures for neck tightening include the Pelleve, ThermiSmooth 250 and EndyMed radiofrequency devices.

Other techniques which complement and can add to neck rejuvenation include procedures such as the Sihouette threads, ThermiRase, fat injections and fillers. Although we offer Kybella, I use that less frequently as personally I find that I can achieve the result the patient is seeking faster and easier with a small liposuction procedure.
