The procedures for fat grafting also known as fat transfer, fat injections or lipofilling have evolved enormously in the last decade. In our practice, fat transfer is predominantly performed using autologous (your own fat) preparation of fat known as microfat and nanofat. Basically, microfat is your own fat which has been mechanically reduced to very small particles of fat which are better suited to pick up a new blood supply and thrive when injected into areas such as the face. Nanofat is produced when microfat is mechanically divided into even smaller pieces to isolate what is known as the SVF or stromal vascular fraction. The SVF contains small cells known as Adipose Derived Stems Cells or ASCs.
Microfat is used when volume enhancement is desired. We frequently use it in our practice around the face to fill in unwanted lines or to replace lost volume in the cheek, along the jaw and chin or in the temporal region. It is also used in the breast to complement the results of implants after augmentation or reconstruction surgery.
Nanofat is not used for volume replacement but instead is used to enhance the quality of the skin by reducing wrinkles and rejuvenating skin by improving the texture and pigmentation.
The procedure for harvesting both microfat and nanofat is done under a local anesthesia. The most commonly used donor sites are the abdomen, inner thighs and love handles. The procedure can take between one to several hours depending on the number of sites to be injected and the volume of fat to be processed.
Because at Rothaus Plastic Surgery we perform the procedure under a local anesthesia, there is almost no recovery. There is minimal to no discomfort and bruising at the donor site. Depending on the volume injected, there may be some short-term swelling and bruising.
